In this practice, Dr. Martell is out-of-network with all insurance plans, and payment is due at the time of service. We will provide a detailed Superbill with all necessary diagnosis and procedure codes (CPT codes), which you may submit for out-of-network reimbursement, or use with your HSA or FSA.
